ED diversion strategies include: - Supporting enhanced clinical knowledge & assessment to identify changes early which leads to early recognition and treatment - Data collection to identify gaps & inform new interventions and policy changes to support ED diversion strategies - Opportunity to work with community partners to visit Labs/DI frequency in the home - Continued analysis important to evaluate success of interventions and highlight new initiatives for continued improvement - Developing partnerships within the community and acute care sector - Palliative care - Specialist care via community partners - Engaging with community partners to facilitate direct admit vs. ED transfer
